($18.17 hour plus 4% vacation pay)

As a successful member of the PSW team you will provide care in the client’s home and community according to Community Care Durham’s policies and procedures while reporting to the Program Manager/Supervisor. You will be required to carry out various tasks as defined within specific services which may include accompanying clients to/from critical primary care appointments and providing appropriate supports and follow-up; providing assistance with daily living such as dressing, personal hygiene, assisting with mobility, assisting and monitoring medication use and other routine living activities; homemaking services including shopping, housecleaning and meal preparation; safety checks and/or reassurance services; transition assistance to clients, including transportation from hospital and settlement services at home and completing necessary client/visit documentation reports.

Qualifications required:

  • Personal Support Worker certificate or acceptable equivalent (e.g. RPN) and experience supporting clients in the community setting
  • Sensitivity to adults with needs related to aging, physical and/or mental health, including challenges in cognition
  • Comfortable working alone in the community with supervisor or service coordinator available on call
  • Satisfactory Police Vulnerable Sector Check
  • Cell phone with internet access and use of licensed vehicle with $1,000,000 public liability insurance; maintenance of a class G Ontario driving licence and a clean Drivers Abstract along with the ability and willingness to drive within Durham Region
  • Physical ability sufficient to assist with client transfers and activities of daily living including toileting and hygiene (able to lift approximately 50 lbs., bend, stretch, stand for extended periods of time, climb stairs, reach, twist, sit, walk and/or run)
  • Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies
  • Availability for shift work, statutory holidays and willingness to work one weekend per month
