Service Coordinator, Home Support Program, Clarington
Temporary Contract up to 12 months – Part Time (14 hours/week)

This position is responsible for coordinating and scheduling the provision of assigned Home Support services and participating as a member of the Community Care Durham team. Working from the Clarington office, the Service Coordinator is responsible for:

A) Coordinating client service provision by:

  • Implementing and evaluating services, in consultation with the Manager, to meet client needs
  • Completing client assessments and follow up as delegated
  • Liaising with various organizations and individuals in the coordination of service
  • Maintaining statistical data, billings and collections

B) Providing volunteer management by:

  • Assisting with recruiting, screening, orienting, training and recognition of volunteers
  • Scheduling volunteers to service requirements considering volunteer preferences and abilities and the needs of Community Care Durham
  • Supporting and monitoring service performance

C) Promoting Community Care Durham services, assisting with office/program/Community Care Durham initiatives and fundraising.

As with any part time position, Community Care Durham reserves the right to alter (increase/decrease) hours of work.

Qualifications required:

  • Post-secondary diploma in social services/human resources and related experience, /or an equivalent combination of education and experience
  • Demonstrated excellent communication and organization skills, initiative, flexibility and accountability for delivering results within deadlines
  • Good knowledge of community support services, scheduling and volunteer management
  • Ability to work in a computerized environment (Intermediate MS Office skills). Knowledge of databases (CIMS etc.) would be an asset
  • Experience with volunteers would be an asset
  • Satisfactory Police Vulnerable Sector Check; licensed, insured vehicle and the ability to travel within Durham Region
  • Sensitivity to adults with needs related to aging, physical and/or mental health
  • Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: $20.72 – 24.00 per hour plus 4 % vacation pay
