Meals on Wheels
CCD’s Meals on Wheels service provides clients and their caregivers with the security of knowing they can live independently in their own home through the delivery of affordable, tasty and nutritious Hot and Frozen meals.
Accessible Vans (AV)
Accessible Vans (AV) transportation service provides wheelchair accessible vans driven by CCD staff to transport clients with mobility issues. Adult Day Program
Are you a senior or adult with needs related to aging, physical and/or cognitive health looking for a social, recreational and supportive atmosphere? Are you presently caring for a loved one and could use an opportunity to take a break? CCD can help!

Assisted Living/Supportive Housing
Community Care Durham’s Assisted Living Services (ALS) and Supportive Housing (SH) services address the needs of high-risk seniors residing at home.

Home at Last
Community Care Durham’s Home at Last (HAL) service supports adults who require assistance to settle back at home after a hospital stay.

Exercise & Falls Prevention
CCD’s Exercise and Falls Prevention Program is a community-based initiative aimed at meeting the exercise needs of Older Adults in the Durham Region.

Foot Care Clinics
CCD’s Foot Care program is an important Health and Wellness service available to people who need assistance with basic foot care due to loss of vision, mobility issues or diabetes or when friends or relatives are not available to help.

Friendly Visiting
Clients are matched with one of CCD’s Friendly Visiting volunteers and receive much needed social interaction, so they can maintain their independence and quality of life while safely remaining in their home.

Telephone Reassurance
This CCD service is ideal for anyone who is not able to leave their own home, socially isolated or lacking caregiver support.

Transportation
Community Care Durham’s Transportation services provide a great solution when you are unable to use public transportation because of needs related to aging, physical and/or mental health and when friends or relatives are not available to help you.

Personal Support (Caregiver Relief) In-Home Respite
CCD’s Personal Support Workers partner with caregivers to ensure their loved ones are safely cared for while caregivers take time for their own activities, helping to prevent caregiver burnout and illness.

WeConnect
WeConnect (formerly COPE Mental Health) is a free community mental health service for people aged 16 and older. We offer group services that provide emotional support, connection with others, education, and strategies to help individuals care for their mental health and overall well-being.
