All CCD programs and vital services continue to help our clients remain safely at home.

Transportation

Transportation

Due to higher than usual Transportation requests, we are now requiring a longer period of time to book your request. We appreciate your patience as we work to resolve this issue.

Community Care Durham’s Transportation services provide a great solution when you are unable to use public transportation because of needs related to aging, physical and/or mental health and when friends or relatives are not available to help you.

Are you in need of a ride?

Rides, primarily to medical appointments, shopping, errands and appointments and to access CCD programs, are provided by CCD staff using CCD vehicles or volunteers using their own vehicles.

To arrange a ride with us to your appointment call 1-888-255-6680 or complete the Request Service form.

Transportation

What Types of Transportation Services are Available

Accessible Vans (AV)

Accessible Vans (AV) transportation service provides wheelchair accessible vans driven by CCD staff to transport clients with mobility issues. CCD staff can push the client up the ramp, secure the wheelchair and bring the client down the ramp. The client must be able to wheel themselves to the ramp or have a caregiver do so. If they cannot travel alone, they must travel with an escort.
Cost: $12. Minimum/$0.55 per km (This is a temporary increase to offset the costs of gasoline.)

Home at Last (HAL)

Home at Last (HAL) transportation service provides Personal Support Worker (PSW) accompanied transportation to bring clients safely home from the hospital if they are aging or have special needs and would benefit from assistance in getting settled.
Cost: There is no charge for this service.

Volunteer Drivers (VD)

Volunteer Drivers (VD) transportation service provides clients with rides to and from essential destinations. Volunteer Drivers are licensed to drive in Ontario, carry their own insurance and are screened through CCD’s volunteer recruitment process. Clients must be able to transfer in and out of a vehicle independently or with light assistance.

Cost: $12. Minimum/$0.55 per km (This is a temporary increase to offset the costs of gasoline.)

Volunteer drivers do not receive payment for their time – this is their generous gift to the community. A per kilometre or flat rate reimbursement is charged for drivers’ out of pocket expenses such as fuel, insurance, and vehicle maintenance.

  • The fee is doubled when the length of the appointment exceeds three (3) hours.
  • Fees for extra stops: $2. Flat fee ( 2 stop maximum, no longer than 15 minutes total)
  • The client is responsible to pay the exact fee in cash at the end of the ride.
  • The client is responsible to pay any applicable parking fees in cash at the time of the drive.
  • Cheques are not accepted. Please no tips/tipping.

Cost

Due to additional expenses caused by the pandemic, we’ve had to make the difficult decision to raise our fees.

The cost varies by the type of transportation service required. A family member or friend is welcome to ride along with you at no additional cost in order to accompany you to your destination.

Where Is Transportation Offered

Rides can take place within Durham Region, to neighbouring communities or across the Greater Toronto Area.

Access to Primary Care is an excellent service, the PSWs are always so willing to help me with my walker and ensure I am able to get to my Doctor’s appointment on time.

The program is so helpful to us as a family. We are not always able to take mom to her appointments and we know that we can rely on the Transportation program to get mom safely to her where she needs to go.
