Community Care Durham’s Transportation Services provide a great solution when you are unable to use public transportation because of needs related to aging, physical and/or mental health, and when friends or relatives are not available to help you.

Depending On Your Needs, These are the Types of Transportation Services Available for You:

Paid Services

Accessible Transportation Service

Accessible Transportation Service is for clients with mobility concerns that require assistance. Our service provides wheelchair accessible vans driven by Community Care Durham (CCD) staff to transport clients with mobility issues. CCD staff can push the client up the ramp, secure the wheelchair and bring the client down the ramp. The client must be able to wheel themselves to the ramp or have a caregiver do so. If they cannot travel alone, they must travel with an escort. Staff are licensed to drive in Ontario and undergo rigorous training and screening before completing a thorough onboarding process.

Cost: $14.00 minimum/$0.55 per km

Assisted Transportation Service

Assisted Transportation Service is for clients who require the assistant of a Personal Support Worker to accompany them to and from a medical appointment and have mobility or other related challenges.

Assisted Transportation Service is suitable for any client who is frail, has special needs, or has needs related to aging or disability; and requires PSW support with mobility or with activities of daily living (meal preparation, personal care, dressing, homemaking, etc.).

This service is provided by certified Personal Support Workers with experience driving clients and the training needed to prepare them for most situations – including First Aid, CPR (Level C), safe lifts and transfers, and safe oxygen handling, among others.

This service is only available for OHIP-funded medical appointments within Durham Region, such as day surgery, including colonoscopy and cataract appointments.

Cost: $35.00 flat fee in addition to the $14.00 minimum/$0.55 per km. All rides are invoiced

Volunteer Drivers

Volunteer Driver Transportation Service is for clients who can transfer in and out of a vehicle independently or with light assistance. Volunteers will load a walker.

Volunteer Driver Transportation service provides clients with rides to and from essential destinations. Volunteer Drivers are licensed to drive in Ontario, carry their own insurance and are screened through CCD’s volunteer recruitment process.

Cost: $14.00 minimum/$0.55 per km

Volunteer drivers do not receive payment for their time – this is their generous gift to the community. A per kilometre or flat rate reimbursement is charged for drivers’ out of pocket expenses such as fuel, insurance, and vehicle maintenance.

  • The fee is doubled when the length of the appointment exceeds three (3) hours.
  • Fees for extra stops: $2.00 flat fee ( 2 stop maximum, no longer than 15 minutes total).
  • The client is responsible to pay the exact fee in cash at the end of the ride.
  • The client is responsible to pay any applicable parking fees in cash at the time of the drive.
  • Cheques are not accepted. Please no tips/tipping.

Free Services

Home at Last (HAL)

Home at Last (HAL) Transportation Service provides Personal Support Worker (PSW) accompanied transportation to bring clients safely home from the hospital if they are frail, or have special needs related to aging or disability, and would benefit from assistance in getting settled.

Cost: There is no charge for this service. This service is arranged by the unit coordinator, social worker, or nurse at the client’s hospital unit. One family member or friend is welcome to ride with a client at no additional cost to accompany them to their destination.

The cost varies by the type of transportation service required. One family member or friend is welcome to ride along with you at no additional cost in order to accompany you to your destination.

Rides can take place within Durham Region, to neighbouring communities, or across the Greater Toronto Area.

How Can I Book a Transportation Service

All rides, including those provided by Volunteer Drivers, must be booked through Community Care Durham. Community Care Durham assumes no liability or responsibility for services undertaken or arranged without the knowledge or approval of the local office.

Access to Primary Care is an excellent service, the PSWs are always so willing to help me with my walker and ensure I am able to get to my Doctor’s appointment on time.
