Home At Last Program (HAL) &
Access to Primary Care (APC)
The Home At Last (HAL) program supports adults who are aging or have special needs and would benefit from assistance in getting settled at home after a stay in hospital. This transition service is provided at no cost by trained and qualified Personal Support Workers (PSW) who arrive at the hospital and drive clients home. The PSW can fulfill client needs like grocery shopping, medication pick-up at the pharmacy, preparing a small meal and providing personal care and/or homemaking services. In certain situations, clients are able to get home on their own but have the HAL settlement service meet them there to provide the supports necessary to ensure their care and safety.
Follow-up is an important component of the HAL program.  Every client receives a follow-up call once they have settled in at home and visits are arranged to check on the client's well-being if needed.  The HAL service also includes making referrals to other community support services when beneficial to the client.  This hospital discharge and follow-up service is offered to those living in Durham Region.  Clients can be brought home from hospitals within or outside Durham Region.

Access to Primary Care (APC)
APC provides PSW accompaniment to high-risk older adults requiring support to attend their primary care appointments or assessments.  PSW staff will accompany clients to their appointment, provide support, and settle the client home safely following their appointment.  This service is coordinated through the HAL Program, and is available throughout Durham Region.
